I understand that by submitting this Unleash’d Membership Cancellation Form, my dog’s membership will be cancelled immediately and my credit card will no longer be billed monthly. A $30 Cancellation Fee will be applied if cancelled before 3-months.

Upon Cancellation(Required)

There is no refund for any unused days left in the current billing cycle. My dog will still receive all the benefits of Membership until this billing cycle finishes. Once the billing cycle ends, all of my dog’s future daycare reservations outside of one-week-out will be cancelled.

For Dogs Enrolled In Enrichment Daycare(Required)

I understand that my dog must be an Unleash’d Member in order to participate in Bark-A-Bout’s Full-Day Enrichment Daycare Program. Once my dog’s Unleash’d Membership is cancelled, my dog will no longer be able to participate in Bark-A-Bout’s Enrichment Program until their Unleash’d Membership is renewed and they are re-enrolled into the Enrichment Daycare Program (by filling out the online application and getting Bark-A-Bout’s approval).
